Galderma Tri-Luma Cream is a potent, prescription-only triple-combination topical medication standardly used for the short-term treatment of moderate to severe melasma of the face.
Active Ingredients Breakdown
The cream achieves its therapeutic effect by combining three separate active components that target hyperpigmentation from different cellular angles:
  • Hydroquinone USP (4% w/w): A highly effective skin-lightening agent. It suppresses melanin production by inhibiting the tyrosinase enzyme, which decreases dark spot formation.
  • Tretinoin USP (0.05% w/w): A vitamin A derivative (retinoid) that increases skin cell turnover. This process exfoliates existing hyperpigmented skin cells faster and allows the hydroquinone to penetrate more deeply.
  • Fluocinolone Acetonide IP (0.01% w/w): A mild corticosteroid included strictly to minimize inflammation, redness, swelling, and itching commonly triggered by hydroquinone and tretinoin.
Clinical Usage & Guidelines
According to the official FDA drug labeling managed by health authorities, strict usage rules apply to this medication:
  1. Duration Limit: It is indicated only for short-term use up to 8 weeks. It should not be used continuously as long-term application can lead to a rare, permanent blue-black skin darkening called exogenous ochronosis.
  2. Nighttime Application Only: Apply a thin layer to the affected dark patches (including 1/2 inch of normal-looking surrounding skin) once daily at bedtime, at least 30 minutes before sleep. Tretinoin breaks down rapidly in sunlight and makes skin highly photosensitive.
  3. Strict Sun Avoidance: Sunscreen of at least SPF 30 or higher must be worn during daylight hours along with protective clothing. Even minimal UV exposure can completely reverse the effects of melasma therapy.
Side Effects & Warnings
  • Common Local Reactions: Redness (erythema), peeling, mild burning, dryness, and itching at the application site are highly expected during the first few weeks.
